Overview
MVRMC's fill-in application form uses the features provided
with Acrobat 4.0 products. There is some validation and verification of the information you enter,
but you are
still responsible for entering all required information correctly (signatures
must be handwritten on the form).
Software Requirements
To view, complete and print MVRMC fill-in forms
you'll need the freely available Acrobat
Reader 4.0 software installed on your computer.
Completing the Form
- Download the form by clicking on one
of the above links or here. Your web browser may be configured with an
Acrobat plug-in to automatically open the file within your
browsers window upon download. To download the file directly to
disk, click on the "Save" icon (
 )
and choose the folder you would like to save to when the menu
window opens, then click "Save".
- Use Acrobat 4.0 to open the file if necessary.
- Select the hand tool from the Acrobat toolbar menu. You can use
the hand tool to move the page around so that you can view all the
areas on it.
- Position the hand pointer inside a form field and click. The
I-beam pointer allows you to type text. The arrow or hand pointer allows
you to select a field, a check box, a radio button, or an item
from a list.
- Press Tab to accept the field change and go to the next field.
- Press Shift + Tab to accept the field change and go to the
previous field.
- Use your mouse to select an area of the form that is not inside
a form field before printing your form. If a form field is
active (contains the blinking bar) the contents will not print.
- When you complete filling in the
required information, print a copy for your records and then
use the "Submit" button at the end of the form to send
to MVRMC's Human Resources Department. If the fill-in form is displayed within your web browsers window
be sure to use the printer button on the Acrobat toolbar menu to
print the form instead of your web browsers print function.

Magic Valley Regional Medical Center is seeking to employ two H 1B
non-immigrants in the occupational classification of Physical Therapists at a salary range of $22.46 to $33.69 for the period from January 2006, to December 2008, at the location of 650 Addison Avenue West, Twin Falls, Idaho. The labor condition application is available for public inspection at the same address. Complaints alleging misrepresentation of material facts in the labor condition application and/or failure to comply with the terms of the labor condition application may be filed with any office of the Wage and Hour Division of the United States Department of Labor.
