MVRMC EMPLOYEE BENEFITS

The following benefits are available to full-time and part-time employees only:

 

 

Health Insurance

$500 Deductible Plan – Pays 80% of the cost of most basic services, including hospitalization (after you meet the deductible). Most preventive services are covered at 100% (with no deductible).

$1500 Deductible Plan - Covers 80% of the cost of most basic services, including hospitalization (after the deductible is met). Most preventive services are covered at 100% (with no deductible).

PPO Plan – A $25 co-payment ($35 Specialist) is required for in-network services such as office visits, well childcare, and routine physicals (no deductible). The plan pays 80% for many other in-network services. Some services are not covered out-of-network.

Dental Insurance

- After a $75 individual/$225 family deductible, the Dental Plan features 100% coverage for preventive treatment, and 50% - 80% coverage for most other procedures. Orthodontia is covered at 50%, with a lifetime maximum of $1,000.

Prescription Insurance

- This plan provides prescriptions at local pharmacy or through the mail order program for prescriptions, (generic, brand name, or formulary).

Vision Insurance

- This plan pays 100% for most vision care services and eyewear if you use network providers, or a reduced price with out-of-network providers.

 

Life Insurance

- Full-time employees automatically receive one times their annual salary (rounded to the next higher $1,000) in basic life insurance/AD&D coverage. MVRMC pays the full cost of this coverage.  Full-time and eligible part-time employees may purchase additional:

  • Supplemental Life Insurance coverage in $10,000 increments.
  • Dependent Life Insurance coverage..$5,000 or $10,000 available.

Accidental Death & Dismemberment

Supplemental AD&D available in $10,000 increments, up to $300,00.  Available for purchase for employees and their families.

Long-Term Care Insurance

Available for employees to purchase for themselves, spouse, parents (in-law), and/or grandparents (in-law).

Long Term Disability

MVRMC provides full-time employees with basic LTD benefits. Upon determination of eligibility, coverage is 50% of your monthly salary, to a maximum of $4,000 per month.

Retirement

Lifetime retirement options beginning for employees at age 55 or 65+, depending on years of service, 5 year vesting.

Adoption Reimbursement MVRMC will reimburse up to $1,000 for eligible adoption expenses for full-time employees who have been employed for at least one year.  

Jury Duty

Full salary for regular scheduled workdays.

Paid Time Off – PTO

From 90 days employment to 4 years accrue 22 days; Employees with 5-9 years of service accrue 27 days; 10 years and over accrue 32 days. Part-time and Full-time accrue on hours worked.

Extended Sick Leave

Employee accrues 64 hours per year, up to 520 hours. Part-time and Full-time accrue on hours worked.

The following benefits are available to full-time, part-time, and PRN employees:

Optional 457 Plans

Safeco/ING/VALIC tax deferred savings plan.
Optional 403(b) Plan  - VALIC tax deferred savings plan.

Cancer/Intensive Care

Specialized insurance at group rates through AFLAC.

Holidays

Time and one-half for working Thanksgiving, Christmas, New Year’s Day, Memorial Day, 4th of July, and Labor Day.

Infant Care Center

Care for infants, age 6 weeks to 18 months.  Payroll deduction is available.

Older Child Care

Payroll deduction for child care is available at Step Ahead and Valley Christian Day Care & Immanuel Lutheran

Hospital Patient Discount

20% discount on unpaid balance of hospital bill after insurance (See Admin Policy).

Private Room at MVRMC

Employees as inpatients at MVRMC will have private rooms (as available).

Cafeteria Discount

Employees wearing identification badge receive discount.

Library Services

Located on the 1st floor, with a wide range of material, as well as periodical searches.

Health Clubs

Payroll deduction for monthly dues to local health clubs.

Travel Discounts

Magic Kingdom Club (Disney) & Getaway Today - Discounted rates on airline, hotel, cruises, car rental, etc.

Employee Assistance Program

Up to five completely confidential visits per incident with a counselor.

Employees Helping Employees

Assistance in time of need through donations given by employees of vacation and sick hours.

Tuition Reimbursement

Maximum of $1,500 per semester for successful completion of classes per criteria. Yearly limit of $3,000. Part-time employees receive disbursement based on hours worked during the applicable period.

Leave of Absence

Family Medical Leave: up to 12 weeks; Medical Leave: up to 1 year; Bereavement Leave: up to 24 hours. Also, Educational, Military, Personal, and Worker’s Compensation Leave available.

Free Yearly Wellness Testing

Cardiac Risk Profile (Cholesterol) and Fasting Blood Sugar.

Free Yearly Mammogram

For female employees over age 40, or employee female spouses over 40.

Some benefits may vary according to Full-time, part-time or PRN status.

If any benefit information listed here varies from the wording in the benefit contract, the benefit will be based on the actual contract.
